The Five-Year Wait

You sponsor a parent, the green card is approved, they arrive at sixty-eight — and then you find out Medicare will not take them for five years, because they never worked here. The statute is unambiguous and the arithmetic is simple. What makes it a trap is that almost nobody meets it until the parent is already in the country. Nothing you enter below reaches any server.

Premium-free Part A is earned by working here — about ten years of it verified

Hospital insurance under Part A comes free of premium to a person who has enough quarters of Medicare-covered employment, conventionally described as forty quarters, or roughly ten years of work in the United States. A parent who spent their working life in India has none of those quarters, so the free route is closed to them from the start. This is not a penalty aimed at immigrants; it is what the benefit is, an insurance entitlement earned by contributions. But it means the ordinary assumption — turn sixty-five, get Medicare — does not describe them.

Without those quarters: permanent residence AND five continuous years before you may enrol verified

The statute allows enrolment for an individual who has attained age sixty-five and is a resident of the United States, and is either a citizen, OR an alien lawfully admitted for permanent residence WHO HAS RESIDED IN THE UNITED STATES CONTINUOUSLY DURING THE FIVE YEARS IMMEDIATELY PRECEDING THE MONTH in which they apply for enrolment. Both halves bind: the green card alone is not enough, and five years of visits is not the same as five years of residence. A parent who arrives at sixty-eight on an approved immigrant visa is not eligible until seventy-three.

Turning sixty-five is not the event — the fifth anniversary of arriving is verified

For a parent in this position the eligibility date is not their sixty-fifth birthday. It is the later of turning sixty-five and completing five continuous years of residence as a permanent resident. Somebody who immigrates at sixty-two waits until sixty-seven; somebody who immigrates at seventy waits until seventy-five. The number that matters is the date of admission as a permanent resident, not the date of birth, and families planning around a birthday are planning around the wrong anniversary.

A parent visiting on a temporary visa cannot enrol at all, at any age verified

The enrolment condition requires either citizenship or lawful admission for permanent residence. A parent here on a visitor visa satisfies neither, however long the visit and whatever their age, and no amount of time spent in the United States on a temporary status counts toward the five years — that clock only runs on permanent residence. Cover for a visiting parent is therefore a different question entirely, answered by private travel or visitor medical insurance rather than by Medicare.

The word is CONTINUOUSLY — and long trips home are the obvious way to break it verified

The statute requires residence in the United States continuously during those five years, not merely five years of holding the status. Extended absences can interrupt that continuity, which matters enormously for exactly this group: a newly-immigrated parent who spends several months a year back in India may be resetting a clock they do not know is running. What length of absence breaks continuity is not stated on this page, because no source consulted stated it authoritatively — that is a question for the Social Security Administration directly, and worth asking BEFORE booking a long trip rather than after.
